Any person wishing to donate securities either through their broker or securities held in certificate form may do so by following these instructions.

 

Transfer of Securities Held By Your Broker

Please follow these instructions

  1. Complete the Stock Donation form and fax, mail or hand deliver to the Foundation.
  2. Contact your broker to initiate the transfer to our depository trust company as follows:

  1. Once confirmation of transfer and sale of stock is confirmed the Foundation will send you an acknowledgement letter of your gift for your records.

Transfer of Securities Held in Certificate Form
(Certificates Physically Held By Donor)

The transfer of physically held security certificates requires two mailings for the Foundation to process your donation.  We ask that you please contact the Foundation office prior to sending any securities and encourage you to mail your documents through registered mail.  If possible we encourage you to personally deliver the stock certificates to our office or we can arrange to pickup your certificates. 

 

Please follow these instructions

  1. Complete the Letter of Authorization and Stock Donation forms.
  2. Mail these two forms along with your security certificates to the Foundation address listed below.  Important:  do not sign the certificates or complete the transfer section of the certificates

  1. Complete the Power form by providing the number of shares, class of stock, name of company and registration numbers listed on the certificates.  The Donor must date and sign the Power form exactly as listed on the certificates.
  2. Mail the Power form in a separate envelope to the Foundation.
  3. Once confirmation of transfer and sale of stock is confirmed the Foundation will send you an acknowledgement letter of your gift for your records.
